Arizona Commission for the deaf and the hard of hearing
The link which I provided takes you to their "referral page". This page has links to information on deafness such as: assistive technology, employment, government agencies dealing with hearing impairment, etc.

Office for Children with Special Health Care Needs

Community Health Program

More information will be added as I locate it. Arizona is giving me some trouble in locating programs. They are there I'm sure, I just have to find them.
